Directions: This form MUST be filled out completely. You must also have one of your coaches this season complete a recommendation letter. Hint, do NOT wait until the last minute to ask your coach. This is a busy time of the year and they will need time to complete the letter. Turn this form AND a sealed recommendation letter into Ms. Flaugher, Director of Athletics, by 9:00 a.m. on Thursday, November 7. Applications will not be reviewed unless complete! Include several paragraphs (50-100 words) about what this award means to you, what it has meant to be an athlete at E.C. Glass, and why you are applying for this honor.

	Have one of your coaches complete a recommendation letter on why you should receive this award. This letter should be sealed and accompany this application. The letter should not be over 100 words.
	All information should be turned into Ms. Flaugher by 9:00 a.m. on Thursday, November 7, 2002.
	Scholarship winners will be announced at the Fall Awards Ceremony which will be held on Thursday, November 21.
